1. Proof of residency, such as a signed lease or rental agreement, utility bill or copy of the school tax bill.

2. Original copy of child's birth certificate, passport with VISA if born outside of the USA.

3. Immunization records from your child's physician.

 4. Custody documents (if applicable).

All of these documents are necessary and mandatory to register your child(ren) for school.

For your convenience, we have posted the required forms under the "Download" link to the right. Please complete one registration packet which includes the Health forms, and a computer privilege form for each child.  If your child is transferring from another school, the appropriate release form (also included in the registration packet) for your child's school level must also be completed.  The Free & Reduced Lunch Application is posted and should be completed if applicable.  If you have any questions, please call.
