Please see Mrs. Wheeler in the guidance office for applications. There are two pages:

1. Physical Fitness Certification (Must have had a physical within one year)
2. Application For Employment Certification (Part I of form only)

Please be sure to do the following when completing the documents:

• Include full name with middle name
• Have a parent or legal guardian sign
• Check your social security number twice for accuracy

Bring completed applications to Mrs. Wheeler in the guidance office where she will print out the appropriate working paper card.

Blue Cards - Age group 14/15      

Green Cards - Age group 16/17

If receiving a Blue card, you must reapply for a Green card at age 16 to be legal.
